Medication Template

Good morning!  I am trying to take this technology thing to the next level by using Google Docs.  I've had quite a few requests for a template of the Medication Chart I use to help manage my husbands drugs.  Here is the link.  http://bit.ly/woGwnh  Hope this helps you out as much as it has us.

Everytime I update the medication chart, which thank goodness is getting less often, I save a new copy in my computer with the date, so I have a record and I print copies for:
  • the wall above where we keep his blood pressure cuff, diabetes test kit, medications and notebook.  The notebook we both use to keep track of which medications was taken when.  Since both of us write this down, it helps things run much more smoothly and medications are forgotten less.
  • a copy for his and my wallet.  This way if he is out and about, which is happening much more often (whoohoo), he has is medication list on his person if he needs to go to the hospital, etc.
  • a copy for which ever doctor we are seeing that week or month, especially the oncologist and his primary care physician.
